📋 Action Plans – Required for Severe Conditions

If your child has a severe medical condition, we require an action plan to ensure the safest care possible at camp.

An action plan is required if your child has any of the following:

Severe food or environmental allergies (especially if prescribed an EpiPen)
Asthma that requires an inhaler
Seizure disorder
Diabetes or hypoglycemia
Any condition that may require urgent medical attention during the camp day

You may submit a form provided by your physician, school, or nurse—or ask us for a blank template if needed.
